Notice For New Patients

Please arrive 15 minutes prior to your appointment time and press the light next to Dr. Gardenswartz’s name. She will pick you up from the waiting room promptly at your appointment time.

There is a parking lot in the building (South of Wilshire on Rexford). There is also 1-2 hour street parking on Rexford and surrounding streets.

Please fill out all of the following forms and bring them to your first appointment. If you are a couple, each person should fill out their own HIPPA and Client Information Form.

Payment Option

If you would like to pay for services online using a credit card, you may use PayPal below. Please add 3% to your bill in order to cover credit card expenses. Click "Pay Now" to use this option and enter to dollar amount. For initial appointments, only checks or cash are accepted, and are due at the time of service.
